Procedure Statement

The associated department(s) must verify student eligibility before enrolling the graduate student in the Graduate Health Insurance Program (GSHIP).

Forms and Instructions

The student should complete the Verification of Student Eligibility Form (Finance Form 1112.1.1f) and submit the completed form to the student's academic department(s). Graduate students confirm their eligibility for the health insurance plan with their completion of the form. The department reviews the form and provides enrollment information for the health insurance plan.
