Chapter 3: Incident Reporting and Related Resources

Introduction

Purpose

This chapter identifies the approved incident-reporting process and related ASOD policies, procedures, forms, and resources. Requirements for submitting, correcting, reviewing, maintaining, and closing incident reports are established in the ASOD Incident Report, Documentation, & Correction Policy.

Scope

This chapter applies to ASOD faculty, staff, residents, learners, affiliates, volunteers, contractors, and other individuals participating in ASOD clinical, educational, laboratory, research, or operational activities. Requirements apply to incidents, injuries, exposures, medical emergencies, near misses, safety concerns, and other reportable events associated with ASOD operations.

Incident Reporting Requirements

Reportable events must be submitted through the ASOD Incident Reporting App in accordance with the ASOD Incident Report, Documentation, & Correction Policy.  Reportable events may include medical emergencies, sharps or bloodborne pathogen exposures, swallowed or aspirated objects, injuries, accidents, near misses, patient or visitor concerns, privacy or information security concerns, equipment or material failures, treatment related events, property damage, and other safety or operational concerns. Uncertainty about whether an event qualifies as an incident should not prevent reporting. Guidance is available through the ASOD Incident Reporting App at https://go.unc.edu/asod-incident-report or ASOD Clinical Compliance.

Immediate care, emergency response, exposure management, patient safety measures, and other required notifications must not be delayed to complete an incident report.
